How to Set up the sRPE & Wellness Report Surveys:

1. Log into your PlayerTek+ online account and select the 'Settings' icon.

2. Click 'Player Profile Settings' from the settings drop down menu.

3. Click the 'Player Logins' tab from the Player Profile Settings menu. 

4. Select appropriate player from the Player drop down menu.

5. Add the appropriate email.

6. Select 'Add Login'

Note: Invite all players that will be required to fill out the surveys. A message containing instructions to register will be sent to the player’s email address.

7. Click 'User Account Settings' from the settings drop down menu.

8. Select the 'Surveys' Tab. 

9. Select 'Squads', 'Players' and 'Surveys' to send to players.

10. Select 'Add Subscription'.

Note: You must repeat these steps for each survey (Wellness Report & Session Performance Survey). You may remove subscriptions by hitting the '-' icon beside the survey.
